Learn about the different types of questions you can include on a Lead Capture Form

Lead Capture Forms are a great way to collect information from new leads and start your lead flow process. They can be embedded on your website, linked from social media, or used inside the 17hats platform.


Standard and Premier Members can use eight types of questions in your Lead Capture Forms. Essentials Members have seven types of questions. All Members can include two kinds of text in Lead Capture Forms.

The two types of text are:

  1. Heading: use this to differentiate sections of a Lead Capture Form.

  2. Test: use this for instructions throughout your Lead Capture Form.

The eight types of questions are:

  1. Short Answer: used to answer a question with one line or sentence of text.

  2. Long Answer: a larger space for leads to write a paragraph or multiple lines of text.

  3. Yes/No: allow leads to choose either yes or no.

  4. Choose from a List: leads may choose one option from a list of options.

  5. Checkboxes: leads may choose multiple options for a list of options.

  6. Date: leads may select a date using a calendar picker.

  7. File Upload: allow a lead to upload a file smaller than 9.4 MB.

  8. *Lead Source: allow leads to select how they heard about you.

*Lead Source Questions are only available for Standard and Premier Members.
